The theme determines the structure and appearance of a WordPress website. The most important piece of advice is quite simple: changes should never be made to the main theme itself, but rather to a child theme. Otherwise, they will be lost with the next update, and updates should never be skipped.
The second question is weight. Many purchased themes include sliders, portfolios, and shop functions that you don't need, yet they still load. A lean theme with targeted extensions loads noticeably less than a jack-of-all-trades.
